(137) PENILE LENGTH BEFORE AND AFTER THE IMPLANTATION OF AN INFLATABLE PENILE PROSTHESIS

Abstract Objectives There is a high satisfaction rate after implantation of an Inflatable Penile Prosthesis (IPP) for patients with erectile dysfunction (ED). The postoperative length of the penis is a factor that can negatively affect this. Therefore, it is important to create realistic expectations. We aim to evaluate the difference in length before and after surgery. Methods We analysed data derived from the ongoing PHOENIX trial. Ethics committee registration number B243201940514. We included 100 patients who were operated from 11-2021 to 09-2022, in a single centre by a single surgeon. Only virgin implants were included. Penile length was measured in a standardized way in the flaccid and stretched state preoperatively. After surgery, the length was measured again in deflated and inflated state. Results Included patients had a mean age of 61 years. A Coloplast Titan prosthesis was used in 51 patients, an AMS CX prosthesis in 46 patients and an AMS LGX in 3 patients. The mean length of the cylinders was 19.57cm. The average length of the penis after implantation was 7.98cm deflated and 8.36cm inflated. The mean difference in length after implantation between deflated and inflated was 0.3cm. Sub analyses of this parameter to compare the Coloplast and AMS group showed no significant difference (0.26cm vs. 0.48cm respectively). After implantation, the length of the inflated state was on average in all patients, 3.3cm longer than the flaccid state preoperatively, compared to only 0.07cm longer than the stretched state preoperatively. For this also no significant difference between the Coloplast and AMS group. Conclusions The stretched penile length preoperatively gives a good estimate of the expected length postoperatively and can be used to give patients a realistic expectation. When the prosthesis is inflated, there is no significant increase in length compared to the deflated state. We found no difference between Coloplast Titan and AMS.
