Abstract Objectives SFPL is the length that a non-erect penis can be comfortably extended to. It has been suggested that SFPL is an excellent surrogate for erect penile length EPL. We aimed to evaluate if the SFPL accurately reflects EPL. Methods We retrospectively review a total of 225 patients from February 2021 to October 2022 who underwent penile ultrasonography with prostaglandin injection (10 mcg). Demographic, vascular and other relevant variables were recorded for analysis. A history of Peyronie disease or penile surgery (excluding postectomy and frenuloplasty) were exclusion criteria. After injecting prostaglandin, measures of the penis were taken (in cm) and only grade 3 of Erection Hardness Score (EHS 3) and 4 (EHS 4) were included. Three different urologists did all the measurements using a caliper, which was placed along the dorsal side of the penis, extending from the pubo-penile skin junction to the tip of the glans where the pre-pubic fat pad was pushed to the bone. Results Mean age 48 (range 17-98). Mean penis size (flaccid) 10.07cm (range 4-17). Mean SFPL 15.32 (range 10.4-19.7). Mean EPL 14.37 (range 9.0-19.5). Mean SFPL (EHS 3) 15.30 (range 10.4-19.6). Mean SFPL (EHS 4) 15.37 (range 11.4-19.7). Mean EPL (EHS 3) 14.05 (range 9.0-19.5). Mean EPL (EHS 4) 14.76 (range 11.0-19.0). Pearson’s correlation coefficient between SFPL and EPL (EHS 3 and 4): r = 0.646 Pearson’s correlation coefficient between SFPL and EPL (EHS 3): r = 0.530 Pearson’s correlation coefficient between SFPL and EPL (EHS 4): r = 0.574 Conclusions A correlation between SFPL and erect EPL has been found. We can consider SFPL a fairly approximate measure in most cases of EPL. Conflicts of Interest Authors have no potential conflict of interest to report.
