Abstract Objectives Research is conducted to evaluate the effectiveness of filler injection techniques for penile augmentation in managing small penis syndrome (SPS) or small penis anxiety. Methods We included 57 patients with SPS elected using Cosmetic Procedure Screening Scale for Penile Dysmorphic Disorder (COPS-P) in the study. Each patient underwent the procedure of penile augmentation with the injection of hyaluronic acid filler in the shaft (10 ml) and glans (4 ml) of the penis. The effectiveness was evaluated with Self-Esteem And Relationship (SEAR) questionnaire before and 3 months after the procedure. Results Regarding the SEAR questionnaire, the satisfaction of sexual life and self-esteem improved significantly compared with the results before surgery by 8.6 ± 0.46 (P <0.001) points. The most serious complications after the procedure we observed in 2 patients: nodal formations and irregular filler distribution. These complications were resolved with hyaluronidase injections. It was reflected in the SEAR scores of those patients and had some effect on overall statistical results. Conclusions The injection method of penile augmentation significantly improves psychosexual status of the patients. It could be considered for the management of small penis syndrome after more detailed research as a safe and effective alternative to conservative approaches to therapy. Conflicts of Interest None
